Cancellation, Etiquette, Policies, Terms & Conditions

*INAPPROPRIATE BEHAVIOR POLICY*
Live Well Massage Therapy has the right to terminate any massage session due to inappropriate behavior or language. Massage therapy is for relaxation and therapeutic purposes only. There is absolutely no sexual component to massage whatsoever. Any insinuation, joke, gesture, conversation, or request otherwise will result in immediate termination of your session and refusal of any and all services in the future. You will be charged the full service fee regardless of the length of your session. Depending on the behavior exhibited we may also file a report with the local authorities if necessary. 

CANCELLATION POLICY
24 hours notice is required to cancel or reschedule an appointment. Failure to cancel appointments at least 24 hours in advance or failure to show up for your appointment will result in a charge of 50% of the scheduled appointment fee which will be processed on the credit card retained on file to reserve appointment.

If you need to reschedule because you are not feeling well, please contact us as soon as possible to reschedule. 

PAYMENT
Full payment is due at time of service. We accept cash, Venmo, HSA & credit cards (Visa, Master Card, Discover or American Express).

ARRIVING LATE
If you are late to your session you are welcome to receive whatever time is left in your appointment. Due to our tightly booked schedule we are generally unable to extend your session beyond your original appointment time. Regardless of the length of the service actually given, you will be responsible for payment of the full service you scheduled.

COMMUNICATE YOUR PREFERENCES
Be sure to speak up. All aspects of your massage session can be modified to your preference: pressure (too light or too deep), lighting in the room, style of music, temperature, and whether or not you choose to have a conversation or enjoy the treatment in silence.

CELL PHONE USAGE
In our effort to maintain a relaxing & tranquil atmosphere for clients, we ask that all cell phones be silenced or turned off upon entering our center.

SCHEDULING
Services are scheduled by appointment only and we do not offer same day appointments. We ask that you schedule with as much advance notice as possible. All appointments are reserved with a credit card at the time of scheduling.

AGE REQUIREMENT
Minors are allowed services with their parent or guardian present. Parents or guardians are required to remain in the treatment room for the full session with their child.

DISCLAIMER 
I understand that the massage therapist at Live Well Massage Therapy does not diagnose illness, disease or any other physical or mental disorder and does not prescribe pharmaceuticals or medical treatment. It has been made clear to me that this massage therapy session is not a substitute for any medical examinations and/or diagnosis and that it is recommended that I see a physician for any physical ailment I may have. Due to the fact that the massage therapist must be aware of any existing physical conditions of clients treated, I have stated any/all of my known medical conditions and if necessary give permission to the therapist to consult with my physician. I assume all risks and perils to my person physically, physiologically or psychologically which may ensue during this or subsequent treatments. As well as any effects deemed detrimental which may ensue afterwards as a result of this or subsequent treatments. I have read all of the above information and fully understand what has been stated and expected from me.  You are agreeing to this once your service is scheduled.
